"Miscommunication"

This situation was handled improperly because of miscommunication. 

Tom is a toymaker. Today Tom came to work tired

As Tom was busy working on his toys, he made a mistake.

He broke the toy.

Tom made a mistake on a toy today.

Samantha told Tom's Boss, Steven, about this mistake.

Steven was not happy with Tom.

Steven goes and yells at Tom about his mistake.

You need to do better! No mistakes!

Steven was not actually that angry but stress in his personal life made him lash out. 

Tom feels bad after Steven yelled at him. He thinks Steven is very mad at him so he stops making as many toys and makes more mistakes. 

"Better Communication"
